Our Vision

People will become knowledgeable about healthy lifestyle practices that will help prevent prediabetes and Type 2 diabetes which will lower the diabetes incidence rate within the county.

Our Mission

To be proactive to prevent prediabetes and Type 2 diabetes by providing prediabetes prevention and screening education for people who have a high probability of being diagnosed with Type 2 diabetes.

Program Overview & Goals

Participants will have opportunity to participate in various innovative educational activities to accomplish the following goals for practicing healthy lifestyles:

Goal I:

People will identify the risk factors and be motivated to take preventative steps through screening, education and practice.

Goal II:

People with family history of diabetes, prediabetes,
gestational diabetes, and overweight will have the
knowledge base and skills necessary to plan healthy meals.

Goal III:

Participants in the Community TAAP program will attend free hands-on cooking sessions.

Goal IV:

Participants will be aware of ways to increase their food resources such as gardening (container gardening and aeroponic gardens), couponing, and grocery sales.

Participants will be empowered to increase
their intake of fruits and vegetables through
social support of the community.
